Describe the item that needs replacement, such as a keyboard, mouse, or monitor.
Include specifications, such as monitor size and type, or type of keyboard or mouse.
If you request headphones or earbuds, specify the type you prefer, such as wired, wireless, or wired over-ear.
For home office equipment requests, include a short business reason for the additional hardware.
If you request equipment for more than one employee, include extra details.
Provide a list of all employees who should receive the equipment and their work email addresses.
List what each person should receive.
Provide the shipping or delivery address.
How to submit a request
Use one of these methods to submit your request.
Email your office manager or IT support.
Submit a ticket in the company ticketing system.
